The LPN supports the Registered Nurse or licensed physician in the provision of patient care, to include delivery of patient care, collection of ongoing patient response to care, discharge planning and patient family education. The LPN works in close collaboration with the RN or a licensed physician as well as the overall Health Care Team to evaluate the patient's plan of care and progress toward goal attainment. All patient care activities are undertaken with the delegation/supervision of a licensed professional. Basic Qualifications:
This position requires graduation from an approved program of professional nursing (LPN). License Required:

This is a full-time position; 40 hours per week; 8:
00AM-5:
00PM; Monday thru Friday. Background screens will be performed and education will be verified prior to employment. Please be prepared to provide required information and/or documentation.
