The Social Worker provides social work interventions for priority Medical Center patient and family needs. 1. Coordinates discharge plans and/or care plans for in/outpatients, including developing plans for ongoing treatment through services and agencies. 2. Performs a psycho/social assessment for continuity of care planning for in/outpatients. 3. Provides short-term adjustment counseling to assist with emotional, social, and economic stresses of illness and disability. 4. Participates as a member of an interdisciplinary team to develop a coordinated care plan for patients. 5. Participates in quality assurance and utilization review to enhance patient satisfaction. 6. Participates in informal and formal consultations; provide, present, and participate in support and training; arrange speakers and develop materials and programs. 7. Participates in Medical Center committees and special projects as assigned. Master's of Social Work from an accredited college or university is required. Also required are:
strong interpersonal skills; a working knowledge of individual and family dynamics; strong organizational skills, an understanding of systems and how to work within systems; an understanding of the impact and implications of disease and the course of disease; excellent communication skills; the ability to perform psycho/social assessments; and a working knowledge of community resources. Licensed Advanced Social Work (LASW) or Licensed Independent Clinical Social Work (LICSW) strongly preferred.
