Visits patients primarily in a Hospital setting or their place of residence.  Assesses patient's mobility, rehab potential and home environment for safety.  Performs comprehensive assessments.  Provides direct physical therapy treatment.  Develops and implements plan of care.  Coordinates care with all other disciplines.  Arranges for equipment and provide training to patients and caregivers.  Accepts and implements orders from physicians.  Participates in team conferences.  Supervises PT Assitant. Possesses a degree from a baccalaureate or masters program in physical therapy approved by an accredited organization. Licensed to practice as physical therapist within the State of California. Two years of appropriate experience as a physical therapist. Hosptial Experience -Preferred.   Demonstrates good verbal and written communication, and organization skills.
