HealthPartners is looking for MN licensed MA,LP's, LICSW's, LMFT's, LADC's, or RN's with mental health certification and 3 years of experience for our Behavioral Health Case Management position at the Bloomington health plan headquarters. Responsibilities include:
provides telephonic care coordination to patients, their families, and providers in addressing behavioral health conditions; educate and empower patients and families to make informed personal health care decisions in order to reach health care goals; and facilitate communication between patient, physician, health plan and community.
